Ashbrook Assisted Living is located at 1121 Fremont Street. It is connected by a link to Cedar Ridge Independent Living. It features 41 apartments with one (1) and two (2) bedroom designs. It has three floors of comfort and security. Ashbrook is private apartments that are designed for seniors that need individually assessed supportive services and plans in order to live independently with the security of 24 hour staffing.

Ashbrook has three levels that includes the basic level. Following is the list of all features of the basic level. (Rent and services included)

Level I ( includes all the basic plus these services )

· 24 hour Staff Available for Unscheduled Needs

· Assistance with 2 showers per Week

· Monitoring of Health Conditions

· Prescription Medication Assistance (with RN or Pharmacy filling a weekly medi-planner if needed)

· Daily Medication Reminders by Staff

Level II ( includes all the basic and Level I plus )

· Therapeutic Bath Once a Week

· Management of Health Conditions Due to Illness

· Up to 1/2 hour of Personal Assistance

· Meal time and Activity Reminders

· Stand by Assist with Independent Transfers

Level III ( indludes basic, Levels I and II

· Personal Assistance up to One Hour Daily

· Limited Respite Services

· Assistance of the one Person with Transfers

· Assistance getting to Meal and Activities
